Protruding mouth is “bimaxillary dentoalveolar protrusion” in medical terminology, which is characterized by unsightly, forward positioned profile line of protruding lips (commonly with receding chins) when compared to the ideal reference line of lower face. Mild protruding mouth cases may be improved by mere orthodontic treatment. However, Moderate or severe skeletal protrusion of the alveolar (gum) bone itself must be treated by surgery, i.e. ASO(Anterior Segmental Osteotomy) What is ASO? ASO(Anterior Segmental Osteotomy) surgery is performed for the patients with protruding mouth of moderate to severe grade. ASO is a surgery that moves the maxillary and mandibular alveolar bone & six teeth complex backwards at once following extracting the (upper and lower) four premolar teeth, under general anesthesia. The surgery time of ASO is only about one hour or less. The most distinguishing feature of ASO surgery from orthodontic treatment is that the A-point can be displaced directly backward. This enables parallel backward movement of alveolar bone without inducing inward slanting of foreteeth which is possibly happens in orthodontic treatment after extraction. The backward movement of A-point is essential to improve upper philtral protrusion Meanwhile, many protruding mouths are accompanied by a short/long or receding/protruding chin. Thus, concomitant genioplasty is frequently performed with ASO in order to complete the ideal profile line of lower 1/3 face. The surgery time of genioplasty is less than 20-30 minutes. Characteristics of protruding mouth 1. The more protruding the alveolar bone itself (i.e. skeletal protrusion) is, the more the philtrum area looks protruding. 2. Lips do not close well, which is called lip incompetence. 3. While lip closing, wrinkles on the chin (so-called walnut wrinkles) occurs, which is called chin muscle corrugation. 4. The vermilions(lips) are thick and everted. 5. Since the philtrum is protruding, the nasolabial folds appear relatively sunken, This is why they look older. 6. Gummy smile occurs when smiling. 7. The lips encroach the straight line connecting the tip of the nose and the tip of the chin. 8. It is easy to look angry or brusque Is extraction orthodontics not enough? If there is moderate or severe skeletal protrusion, orthodontic treatment with extraction is insufficient. In these cases, orthodontics cannot complete the ideal lower facial contour. The term “orthodontics for protruding mouth” can easily be misunderstood as meaning that all protruding mouth can be completely improved with orthodontics only. However, not all protruding mouth can be completely improved even with extraction and orthodontic treatment for more than 2 years. Moderate to severe skeletal dentoalveolar protrusions require ASO surgery. This is because the skeleton around the A-point and ANS cannot move directly backward with orthodontics alone. Another limitation of orthodontics is that the chin position & length cannot be adjusted at all by the orthodontic treatment. Preoperative examination and analysis By one-stop visit, our co-work orthodontist establishes the preoperative surgical plan through correct analysis of protruding mouth including the chin position 1) patient interview and X-ray (Cephalometry, Panoramic view, CT) analysis is performed. 2) Blood tests, electrocardiograms, etc for general anesthesia take about 15 minutes. 3) The surgery can be performed 2-3 days after full examination in general. Exceptionally, surgery on the day of examination is also possible 4) Refined and/or 3-dimensional movement of ASO is planned to fulfill the patients’needs. 5) The position of the chin is also important. Chin position is also diagnosed and analyzed before surgery. Forward/backward movement and/or shortening/lengthening may be necessary. 6)The final contour is regulated delicately during surgery. ASO surgery vs Double jaw (bi-jaw ; two jaw) surgery Double jaw surgery is to treat prognathism. It is composed of LeFort surgery of maxilla and setback surgery of mandible(SSRO; Sagittal Split Ramus Osteotomy of mandible) Unfortunately, many protruding mouths are incorrectly diagnosed as candidates for double jaw surgery In case of double jaw surgery, postoperative IMF(Inter-Maxillary Fixation) by rubber band may induce unexpected airway obstruction or asphyxia. The double jaw surgery has relatively wider operation field, relatively greater possibility of major vessel & nerve damage. On the other side, in case of ASO, there is no IMF following surgery, which enables comfortable talking and breathing without any risk of airway obstruction or asphyxia. In ASO, the surgical area is limited, bleeding is relatively less, there is no deep osteotomy close to the alveolar nerve passage, and the surgical time is shorter. The patients can usually return to their daily life 3-4 days after the surgery. Why you can trust SeoulCheil Plastic Surgery. 1. Humanism No matter how much a doctor has aesthetic sense, skilled surgical skills, and craftsmanship, if he does not have a warm heart, there is a very high possibility that the surgery will become a tool for making money. CCTV during surgery We routinely record the operating room with CCTV. If the patient or the patient's family wants to see the CCTV, we will confidently show it to you. Simultaneous facial bone surgery If the protruding mouth is accompanied by prominent cheekbones and/or square jaw, ASO can be performed with malarplasty and mandibuloplasty at the same time. Furthermore, eye and nose surgery can be added. In general, malarplasty and mandibuloplasty is easier surgery than ASO. The total surgery time for the protruding mouth(ASO) + cheekbones(malarplasty) + square jaw surgery(mandibuloplasty) is approximately 3 hours. Following all these three simultaneous surgeries, the patient is hospitalized for only one night, without any drainage bags (Baro-vac; Hemo-vac). 11.
